In order to finish your Air Force ROTC scholarship application, you must download, complete and submit/upload the following forms (see below) to us no later than January 11, 2013.

Forms that need to be submitted:

  1. Counselor Certification/Signed copy of Transcript (9th-11th grades only)
  2. Extracurricular Activity Sheet
  3. GPA and SAT and/or ACT scores
  4. Physical Fitness Assessmentarrow-glossaryPhysical Fitness AssessmentAn exam consisting of three events: crunches, push-ups and a 1.5-mile run. Important information about required forms:

  1. Counselor Certification/Signed Copy of Transcript
    Download, print and submit the form to your counselor for review/approval and request a copy of your transcript (May be unofficial or official copy with 9th–11th grades only—DO NOT wait for your 12th-grade grades as they are not considered.). Scan and upload signed counselor certification form and signed transcript. These forms are required in order to maintain your eligibility. Faxed copies WILL NOT be accepted.
  2. Extracurricular Activity Sheet
    Please complete and submit the form online after completing your scholarship application. Faxed copies WILL NOT be accepted.
  3. GPA and SAT and/or ACT scores
    You must have an ACT composite of 24 (Writing Test not included) or an SAT score of 1100. (We do not mix and match scores, and the Writing Test is not included.)

    Scores must be submitted/uploaded by January 11, 2013, under the following —

    • Electronically through the ACT or SAT agency by using AFROTC test code 0548. (See note below.)
    • You must also print them off the agency websites (www.collegeboard.com or www.act.org) and submit/upload them, even though they may say "unofficial."
  4. Physical Fitness Assessment (PFA)
    Print a copy of the Physical Fitness Assessment (PFA) work sheet. Take the work sheet with you when you complete your fitness assessment. Have the examiner fill in all of the information on the form and make sure he/she signs and dates the form.

    After you have completed the PFA, use the work sheet (filled out by your examiner) to enter your results electronically in the fields provided. You will also need to scan/upload the completed and signed PFA form. Faxed copies WILL NOT be accepted.

    NOTE: We will not accept any PFA scores/test from any other branch of service to include the United States Air Force Academy (USAFAarrow-glossaryUSAFAUnited States Air Force Academy ). Also, if you live in an area where the weather is bad in the winter, please do not wait until the deadline is approaching to take the test. There will be no exceptions made for extending the deadline due to inclement weather.

  5. Résumé

    Please complete your résumé online after completing your scholarship application. Faxed copies WILL NOT be accepted.

    Required Forms Notes

    • Letters of recommendation will not be considered as part of the Air Force ROTC scholarship selection board. Please do not submit these with your application.
    • Ensure that all correspondence includes your name and Social Security number.
    • We highly encourage you to make a photocopy of your score report with the counselor certification, signed copy of transcript, PFA results, etc., for your records.
    • It is your responsibility to verify that all required documents are submitted/uploaded by January 11, 2013, to Headquarters Air Force ROTC/RRUC. Check the online application tracker or voice-automated system or contact your scholarship technician directly to determine the status of your application.
