You cannot access the forms unless you completed an online application by December 1, 2010.
In order to finish your Air Force ROTC scholarship application, you must download, complete and mail the following forms (see below) to us postmarked no later than January 8, 2011. However, certain items can be faxed, and specific instructions for each form will be supplied once you have completed the online application in a separate email. We highly recommend you send mail certified with a return receipt, as our office is not responsible for lost mail.

In order to complete the Air Force ROTC Scholarship Application, you will need to download and complete the forms after completing the online application. If you need help downloading these forms, please see the instructions below.
Important Information About Required Forms
- Counselor Certification
Have your counselor fill out the counselor certification. The extra curricular activity sheet must accompany the counselor certification form. This form is required in order to maintain your eligibility. Faxed copies WILL NOT be accepted. - Personal Statement - one page only
Fill out the personal statement. Faxed copies WILL NOT be accepted. - Physical Fitness Assessment (PFA)
Applicants must complete the PFA Letter of Certification (we highly recommend you do your best). Please do not send your PFA until it has been completed and certified for each event. This form may be faxed.
NOTE: We do not accept any other PFA results from USAFA
USAFAUnited States Air Force Academy or any other branches of service.
- Résumé
You must also complete a one-page résumé highlighting your accomplishments, focusing on your proven leadership abilities. Please see example résumé for proper format. PDF MSWORDAny résumé received that is longer than one page will NOT be accepted - NO EXCEPTIONS.
This form may be faxed.
Required Forms Notes
- Letters of recommendation will not be considered as part of the Air Force ROTC scholarship selection board. Please do not submit these with your application.
- Ensure all correspondence includes your name and social security number.
- We recommend that you send all these items at the same time in a single envelope. We highly recommend you send mail certified with a return receipt, as our office is not responsible for lost mail.
- We recommend that you make a photocopy of your score report with the counselor certification, personal statement, PFA results, résumé, etc. for your records.
- It is your responsibility to verify that all required documents are postmarked by January 8, 2011 to Headquarters Air Force ROTC/RRUC. Check the on-line application tracker, voice-automated system or contact your scholarship technician directly to determine the status of your application.

All of the files are available in Adobe Acrobat Reader Portable Document Files (PDF) and Microsoft Word (MSWORD) documents. Here's what you need to do to download the PDF documents.
- Step 1: Download and install Adobe's free Acrobat Reader.
- Step 2: Click with your RIGHT mouse button on the file title and choose "Save Target As" and save the file to your computer. Or click on the file title with the LEFT mouse button, view the file and then click on the “save” icon in the Adobe Reader Window.
Here's how you download the MSWORD documents.
- Step 1: Click with your RIGHT mouse button on the file title and choose "Save Target As" and save the file to your computer. Or click on the file title with the LEFT mouse button, view the file and then click on the “Save” icon in the Microsoft Word Window.
Along with the required forms, you must also send:
- An Official high school transcript with a raised seal or signature from the guidance counselor. Faxed copies will not be accepted.

You must have taken the SAT or ACT by 1 Dec 10. You must also have an ACT Composite of 24 (Writing Portion not included) or an 1100 SAT Score (We do not mix and match scores and Writing Portion is not included).
Scores can be sent in using the following and must be postmarked or faxed by 8 January 2011 -
- Electronically through the ACT or SAT Agency by using AFROTC
AFROTCAir Force Reserve Officer Training Corps Test Code 0548. (See note below) - By mail, fax, or they can be put on the official high school transcript. You may print them off the agency websites (www.collegeboard.com or www.act.org) and we will accept even though it may say unofficial.
NOTE - We highly recommend even if you have decide to have scores sent electronically to follow-up using mail, fax, or on high school transcript as well.
